- Double- sided photocopy of marriage declaration or notarization for cohabiting couples
- Blood screening tests by the interested person :
- Check up for HIV I-II, HbsAg, anti-HCV, VDRL (syphilis)
- Covid – 19 PCR couple’s/single woman’s test test 2 days prior to the egg collection or semen collection

- Hemoglobin electrophoresis, Full Blood Count (FBC), iron (Fe), Ferritin
- rubella antibodies (IgG, IgM)
- toxoplasma antibodies (IgG, IgM )
- Cytomegalovirus (CMV) antibodies (IgG, IgM)
- Cystic fibrosis(CF) 100%
- Karyotype
- Thrombophilia
- Natural killer cells (NK Cells)

- Sperm DNA fragmentation (in cases for oligoasthenozoospermia)
